Private Discharge and Reablement Service

This unique collaboration aims to provide therapeutic reablement care to clients exactly when and where they need it.

Our service emphasises flexible, person-centred therapeutic care delivered by specialist Physios and Occupational therapists, supported by fully trained therapeutic care assistants. This innovative service seeks to support the healthcare system within both the private sector and the NHS.

What is All At Home?

We’ve really tried to create this service from a patient’s perspective, thinking about what they need when they are in a state of transition i.e. hospital discharge, leaving respite, recovery from temporary illness/injury at home.

They all need something different, but it would all encompass a level of care and rehabilitation.

 

GoodOaks Homecare – East Dorset

The care element is provided by GoodOaks Homecare – East Dorset – We can do anything from 30 mins to 24 hour care (including double-ups) on discharge from hospital or respite and this will be reviewed on a weekly basis with the goal of reducing the care need and maximising independence.

Medella Home Physiotherapy

The rehab aspect: The carers or therapeutic care assistants, are trained by the highly experienced and passionate OT’s and physio’s from Medella Home Physiotherapy to be enabling in their approach of all functional tasks (both personal care and household related) and to support the patients in doing their exercises and functional activities as directed by OT/PT.

The patients will all receive one face to face session per week of qualified therapy input as part of the package (more is available if needed at additional cost) and a qualified OT/PT will also be reviewing the caseload and supervising the All at Home care team on a weekly basis (plus on demand).

Individualised and Flexible Packages

We understand that each client has unique needs. Therefore, we tailor our care packages to provide the reablement necessary for clients to regain their safety and independence at home, at their own pace.

Care packages can be single-handed or double-handed and range from 30 minutes per day to 24-hour care at home.

We recognise that the needs of a person just leaving hospital are likely to change as they recover and rehabilitate at home, so the package is reviewed and adapted on a weekly basis, with the goal of reducing the care need and maximising independence.

We don’t just provide care and therapist-led rehabilitation, we can also support with companionship, shopping, community access and domestic tasks as the client requires.

 

How long can the service be used for?

There is no set timescale for the service, other than a minimum of 2 weeks. There would be a care review with the patient/family at 6 weeks unless requested/needed sooner.

When the patient finishes with the service, they can continue with care (if needed) from GoodOaks or whichever care provider they wish.

Physio and OT can stop or be continued separately as the patient needs.
